Orientation Forms

Incoming 9th &  New Students :

Complete the Demographic Form & Parent Portal Authorization. 

The Media Release form is only completed if you would like to opt your child out of all forms of media at the school. 

All Returning Students

You are only required to provide a copy of the immunization card, every year. 

Optional: 

1) Photo Consent  OPT OUT form. 

2) Demographic form ONLY IF updates are needed on your child's PowerSchool account.
